Outreach and Support Services

Outreach and Support Services are provided to men and women with a history of chronic homelessness and/or life circumstances that threaten their ability to maintain permanent housing without support.

Services provided include ongoing entitlement and benefit management, and referral to and maintenance of linkages with supportive services for counseling, mental health issues, or substance abuse, transportation and a meals/food program. Outreach services are provided for as long as needed to maintain housing stability, engage in shelter services, or to prevent a crisis episode.
